For an extra layer of identity protection, you can have a law firm set up the DAF on your behalf, then even the fund manager/bank will not have knowledge of the source of the funds. The fund manager at the bank will then send a check to CHD using this fund name or indicate that the grant is made at the suggestion of an anonymous donor.
